• Providing and directing provisions of nursing care to patients in their homes as prescribed by the physician
• Coordinating total plan of care with other health care professionals involved in care
• Making initial and/or comprehensive nursing evaluation visits and developing plans of care
• Ensuring patients meet home health eligibility and medical necessity guidelinesRequirements for this role include:
  • Current RN licensure in the state of practice
  • Current CPR certification
