Job Description:

We are hiring for an RN Case Manager in Hospice. At Heart of Hospice in Fort Smith, a part of LHC Group, we embrace a culture of caring, belonging, and trust.You can expect to develop trusting relationships as an end-of-life care expert, being valued and respected by patients and their families, employee-focused wellness and support programs, and incredible team support and empathetic leadership.As the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered.You will make the initial nursing evaluation in determining eligibility for hospice services, identify patient needs, initiate and coordinate the plan of care, and document patient progress.Requirements include one year of clinical experience, current RN licensure in the state of practice, and current CPR Certification.
