Job Description:
A company is looking for a Telephonic Health Services
Coordinator responsible for patient outreach, registration, and
providing telephonic interventions for patients with chronic
conditions.
Key Responsibilities
Conduct outreach to prospective patients and facilitate their
enrollment in Remote Patient Monitoring services
Collaborate with the care team to support patients in achieving
their health goals
Document patient information and maintain clinical charts according
to organizational standards
Required Qualifications, Training, and Education
Preferred degree from accredited medical assisting programs or
equivalent experience
6 months of experience as a Medical Assistant or 3 years in a
healthcare setting with direct patient contact
Strong computer skills and ability to navigate multiple
programs
Knowledge of medical terminology and experience with electronic
health record systems
Active and in good standing CNA Certification or equivalent
